Personal Medical Records

Every time you visit your doctor, hospital, or other healthcare provider, staff at that centre will make a record of your visit. This information is then compiled into what is known as your medical, or health, record and the information is used by doctors, nurses, and other medical staff in that particular healthcare or medical centre to ensure you receive quality healthcare.

Description:   The May 2008 version (reprinted 2009) of the NSW Child Personal Health Record (PHR) incorporates a number of changes which were identified through a process of clinician and parent review. The printed version, distributed at birth to each baby born in NSW, includes the Parent Evaluation of Developmental Status (PEDS) in each health check from 6 months. The PEDS does not appear in the electronic version.
